Applications for grants will be accepted only by submissions made by email to: [email protected] following the process described below.

Request For Application Form

There is a multi-step process for grant applicants. The first step is to submit a Letter of Intent to enter your organization into the grant application process, no later than August 15. Step two is to submit a formal application no later than October 1. This should be sent via the application contact from on our “Application page.” The form will request, among other things, the following information:

  1. Name, address, ward number (if applicable), website, and telephone number of the applying organization and the name of the person the Foundation should contact concerning the application;
  2. An opening summary paragraph that describes the proposed project, the amount being requested, and the period of time covered by the grant;
  3. A statement of need for the project;
  4. A description of the proposed project activities and desired outcomes, including the geographic area that will be served and the constituencies that will benefit from the project;
  5. The experience and credentials of the organization that make it the best organization to undertake the proposed project, including its tax-exempt status;
  6. How the funds will be used and for what purposes;
  7. A copy of its ruling letter from the Internal Revenue Service stating that it is not a private foundation under paragraphs (1) or (2) of section 509(a) of the Internal Revenue Code;
  8. Current year operating budget;
  9. A program or project budget, and; 
  10. A list of all its directors, officers, and key employees. 

The Foundation’s leadership team will review the application form and potentially initiate a third step. If the Board would like to receive additional information or schedule a site visit, the prospective grantee will be sent a followup email.

After the Foundation has received a complete proposal with all the required information, the grant request will be submitted to the Foundation’s Board of Directors for consideration. Each applicant will be notified of the Board’s decision.
